Facility Use Request
Please submit ONE MONTH IN ADVANCE of event. Submission of this request does not guarantee approval of this event. You will be notified if this request is approved and asked to completed a “Facility Use Agreement”, Pay a Deposit, and Submit Proof of Insurance. Failure to return these additional items within 1 week of the notification of approval will result in cancellation of your event.

- Pathway Newsletter: 15th of the month prior
