Facility Use Request

Please submit ONE MONTH IN ADVANCE of event. Submission of this request does not guarantee approval of this event. You will be notified if this request is approved and asked to completed a “Facility Use Agreement”, Pay a Deposit, and Submit Proof of Insurance. Failure to return these additional items within 1 week of the notification of approval will result in cancellation of your event.

    We will be serving food (FUMCCV is not responsible for providing any paper products, utensils & serving ware)We will be cooking onsite (paid caterers must have liability insurance to cook onsite)We would like our event set-up by custodial staffWe will need childcare
